    Plans, organizes, and conducts dietetic education for nursing personnel and other personnel. Functions as a member of the interdisciplinary team with expertise in nutritional care regarding appropriate hospice patients.

    1. Develops and implements a nutritional plan of care which is based on a comprehensive assessment of nutritional needs as part of the patient's total plan of care. Evaluates and reports results appropriately.
    2. Works with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families/caregivers in the home setting.
    3. Evaluates and instructs on nutritional principles, dietary plans, food selection and economics, and adapting plans to the individual's illness, as indicated. Re-evaluates nutritional care by providing home follow-up care either at the patient's residence or by phone.
    4. Communicates appropriate dietary history and nutritional care data through written record systems.
    5. Records subjective and objective information, assessment, treatment plan, and current progress notes in the patient's clinical records including a discharge summary.
    6. Attends interdisciplinary group meetings.
    7. Complies with the organization's policies and procedures. Maintains confidentiality and safety of patient records so that no information is divulged to other than proper users.
    8. Actively participates in quality assessment performance improvement teams and activities.

    Education and Experience
    Bachelor’s Degree in Dietetics, Nutrition Science or a related field and successful completion of an internship; or equivalent experience.


    Certifications, Licenses, and Other Special Requirements
    Must be currently licensed by appropriate states as a Registered Dietician. Requires membership and registration in the American Dietetic Association. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

    Applies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these.


    Knowledge and Skills
    Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Thorough knowledge of principles and practices of nutrition including the nutritional problems of a variety of age, cultural, social and economic groups; principles of program planning, implementation and evaluation. Knowledge of corporate business management, governmental regulations and CHAP standards. Demonstrates good communications and public relations skills. Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities. Understands hospice philosophy.
