    Provides direct care to residents following an individual service plan, treats each resident with respect and dignity, recognizes individual needs, and encourages independence. Fosters a homelike atmosphere throughout the community. Serves as a resource to other staff members. May serve as "on call" and responsible for providing staffing coverage.


    1. Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual service plan. Allows and encourages residents to do as much of their own care as possible.
    2. Serves meals to residents in the dining room or apartments. May assist in preparing meals following preplanned menus. Encourages residents to eat a proper diet and take adequate fluids while respecting the resident's right to free choice. Records and reports changes in resident's eating habits.
    3. Initiates and participates in leisure activities provided for residents as described in the activity calendar. Encourages residents to socialize and participate in planned activities or programs and to develop friendships with other residents.
    4. Maintains residents' records daily in a timely manner and in accordance with company policy and procedures. This includes documentation related to: medication distribution; leisure activities; incidents and observations; charting any changes in resident's physical condition and/or behavior; visitors; etc. Observes and documents the health and emotional condition of each resident.
    5. Assists residents with medication as defined in medication procedure. Assists or supervises residents who self-administer medication. Follows physician orders and state laws to administer medications to residents who cannot self-administer their medications. Exhibits understanding of and follows policy and procedures for medications. (Refer to state specific regulations for assistance with medication).
    6. Maintains a clean, safe, and orderly environment for the residents. Performs general housekeeping; following cleaning schedules for laundry, bedrooms, dining area, living space, bathrooms, kitchen, etc.
    7. Follows proper procedures in emergency situations and responds promptly to resident requests for assistance - including emergency pull cords, telephone calls, and requests from family and friends.
    8. Knows location of each resident, reminds them to sign out when they leave the building, and notifies the supervisor immediately if unable to locate a resident.
    9. Contacts other care givers to fill vacancies in the schedule.
    10. Responsible for medication administration as allowed by law or regulation by weekly verification that self-medicating residents are taking meds (and are completing the medication administration record, where applicable). Assists with review of medication book proper documentation and for errors. Reviews medication error forms and obtains the appropriate staff signatures before forwarding to supervisor. Responsible for PRN administering medication reorders.
    11. Assists supervisor with stocking medical and office supplies on a weekly basis, following up weekly on bath schedules, blood pressures, and weights, and making new ADL and activity sheets.
    12. Assumes supervisory role in the absence of community managers including the direction of resident care staff problem resolution.
    13. Follows up on resident concerns as needed and directed by supervisor.
    14. Assists staff with training/orientation. Encourages teamwork through cooperative interactions.

    Education and Experience
    High school diploma or general education degree (GED) preferred; and three to six months related experience and/or training; or equivalent combination of education and experience. Familiarity with Microsoft Windows, Microsoft Office and E-mail. Basic typing skills are essential. Knowledge on how to use a mouse, printer, scanner and fax machine is desired. One year work experience in direct service with older adults and knowledge of dementia, particularly of Alzheimer's type, is helpful.


    Certifications, Licenses, and Other Special Requirements
    In accordance with state law, may need to possess current state certification and follow regulations to maintain certification currency. Refer to state specific regulations for qualifications required to assist with medications and direct care duties.

    Uses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally


    Knowledge and Skills
    Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence.  Ability to effectively present information in one-on-one and small group situations to residents, residents' families, and other associates of the company. Ability to effectively communicate in English and understand and follow written and oral instructions. Uses good judgment in emergency situations, and gets help when needed.
