Leads the Intake Nurses and clerical staff within Centralized Intake Services. Ensures that the Centralized Intake staff comply with policies and procedures developed, and maintain expected standards for productivity and customer service.

Education and Experience
Diploma or Associate’s Degree in Nursing plus a minimum of 2 years of home health experience, with a minimum of two years of experience in a supervisory capacity. Bachelors’ Degree preferred. High level knowledge of home health referral process. Confident and passionate about advocating for patients care, coordinating care and communicating patient needs to the interdisciplinary team. Excellent interpersonal, communication, and negotiation skills. Basic computer skills, including Microsoft Office Suite products required. Demonstrated knowledge of Federal and Local standards, as well as Regulatory compliance with Home Health and Hospice Conditions of Participation (CoPs) and Local Coverage Determinations (LCDs) as related to Intake functions. Demonstrated ability to manage defined systems and processes within Brookdale Healthcare Services and successfully convey knowledge. Possesses excellent oral and written communication and teaching skills.
Certifications, Licenses, and Other Special Requirements
RN with current, active and unrestricted license in the state of Tennessee. Must be able and willing to obtain and maintain additional licensure in the state(s) of assigned territory.
Uses independent judgment to make decisions based on establish procedures. Solves problems using standard procedures and precedents. Knows when to refer issues to management and when to handle them personally.
Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Must be able to work under stress and in a fast-paced environment. Demonstrates professional conduct and ethics according to agency policy and procedure. Demonstrates compliance with agency policies and procedures, and federal, state, and local regulations. Must maintain confidentiality of patient information and agency operations.
