    Leads the Intake Nurses and clerical staff within Centralized Intake Services. Ensures that the Centralized Intake staff comply with policies and procedures developed, and maintain expected standards for productivity and customer service.

    1. Provides support and assistance for Centralized Intake staff as needed to ensure an efficient operation.
    2. Maintains presence on intake floor at all times to answer questions related to orders and other clinical intake issues, provide support/guidance and answer questions related to referral needs.
    3. Assists clinical staff in troubleshooting clinical questions and/or concerns in regards to referral content.
    4. Provides team coverage based on referral volume and/or staffing as needed.
    5. Participates in “on call’ schedule for Central Intake clinical coverage on weekends and holidays to meet the needs of the networks.
    6. Acts as mentor for new Centralized Intake Associates related to clinical questions.
    7. Provides updates on staffing needs, training needs and agency-specific requirements to Intake Director to ensure efficiency of operations.
    8. Participates in Quality Improvement activities as requested to ensure ongoing success of and revisions to processes and procedures.
    9. Provides feedback and recommendations for process revisions based on agency and patient care needs.
    10. Stays abreast with updates to computer software programs as they relate to Intake process.
    11. Acts as software and process “super-user” to support ongoing training.
    12. Communicates and maintains relationships with referral sources, attending physicians, Brookdale associates and other agencies as needed to coordinate initiation of services and ensure a seamless transition.
    13. Maintains direct communication with the Director of Clinical Services (DCS) and Clinical Services Manager (CSM) regarding referrals and potential admissions.
    14. Provides daily reporting to care team regarding pending admissions, new admissions, and outstanding (open) items.
    15. Adheres to communication guidelines established by Brookdale and local and regional management.
    16. Provides professional customer service to associates, patients, physicians and other referral sources.
    17. Maintains a safe and clean work area and adheres to facility/corporate safety standards.
    18. Promotes commitment to Brookdale standards, systems, policies, and procedures. Adheres to established confidentiality standards and promotes the standards are maintained by all regional office and field level associates.

    Education and Experience
    Diploma or Associate’s Degree in Nursing plus a minimum of 2 years of home health experience, with a minimum of two years of experience in a supervisory capacity. Bachelors’ Degree preferred. High level knowledge of home health referral process. Confident and passionate about advocating for patients care, coordinating care and communicating patient needs to the interdisciplinary team. Excellent interpersonal, communication, and negotiation skills. Basic computer skills, including Microsoft Office Suite products required. Demonstrated knowledge of Federal and Local standards, as well as Regulatory compliance with Home Health and Hospice Conditions of Participation (CoPs) and Local Coverage Determinations (LCDs) as related to Intake functions. Demonstrated ability to manage defined systems and processes within Brookdale Healthcare Services and successfully convey knowledge. Possesses excellent oral and written communication and teaching skills.


    Certifications, Licenses, and Other Special Requirements
    RN with current, active and unrestricted license in the state of Tennessee. Must be able and willing to obtain and maintain additional licensure in the state(s) of assigned territory.
