Overview

 

Brookdale is seeking a compassionate and adaptable Travel Nurse  (LPN) to join our dedicated clinical team, providing top-tier care across two of our vibrant assisted living and memory care communities—Brookdale Jones Farm in Decatur, AL and Brookdale Hampton Cove in Huntsville, AL. This unique position offers the opportunity to work collaboratively across both locations, supporting residents with a variety of healthcare needs while gaining exposure to diverse clinical environments.

As a Travel Nurse, you'll ensure continuity of care, respond to resident needs, assist with medication administration, and support resident wellness programming—all while enjoying the flexibility and variety that comes with a travel assignment.

 

Key Responsibilities:

  • Deliver high-quality clinical care and services to residents, aligned with Brookdale’s person-centered philosophy.
  • Travel between Brookdale Jones Farm and Hampton Cove locations based on need.
  • Monitor residents’ health status and update care plans accordingly.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and communication.
  • Support medication management and assist in staff training as needed.
  • Provide emotional support and engagement to residents and their families.

Responsibilities

Provides care to residents in accordance with physician orders, recognized standards of practice and established company policies and procedures. Travels to various communities throughout district or state on a rotating schedule to cover temporary vacancies or to provide additional support.

  1. Assists in maintaining a physical, social and psychological environment in the best interests of residents. Ensures resident rights are maintained at all times.
  2. Monitors residents within state licensure regulations; supervises and directs nursing assistants where allowed by state licensure regulations.
  3. Assists in writing care plans based on resident needs and change of condition.
  4. Obtains and administers medication and treatments as prescribed by physician. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policy.
  5. Monitors responses to care treatment plans. Possesses knowledge of medication interactions and notifies Nursing Supervisor or designee of any issues. Adheres to company systems, processes and procedures.
  6. Counts controlled substances with off going nurse or designee; secures and corrects reconciliation of controlled substances during shift.
  7. Ensures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations and company policy.
  8. Assists with discharge process relevant to medication and treatment orders.
  9. Possesses knowledge of OSHA regulations, infection control standards, and emergency procedures.
  10. Follows policies and procedures to ensure resident safety.
  11. Documents all pertinent information regarding nursing care, care plans and observation of resident overall condition.
  12. Attends training to maintain skill level and remain current in emerging healthcare trends.
  13. Tracks mileage and other relevant travel expenses pertaining to assignment.

Qualifications

Education and Experience

  • Education as required to obtain state nursing license (LPN/LVN) and/or minimum of one year related experience and/or training;
  • Additional education can be substituted for experience on a year for year basis.

Certifications, Licenses, and Other Special Requirements

  • LPN or LVN license.
  • Basic CPR/first aid training.
  • Fire safety training.
  • Valid Driver’s License
  • Depending on location of assignment, may be required to temporarily stay onsite at community or in alternate lodging provided by company.

Uses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

 

Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Familiarity with Microsoft Windows, Microsoft Office and e-mail (Microsoft Outlook or Exchange, preferred). Basic typing skills are essential. Ability to maintain resident confidentiality.
