Completes functions related to the intake process for Home Health/Hospice patients for assigned geography including workflow within the Electronic Medical Records (EMR) system. Responsibilities include, but are not limited to, managing the intake process for Home Health/Hospice, setting up patient account information, and facilitating face-to-face physician encounter visit.

Education and Experience
High School Diploma required. A minimum of two years of experience in an office setting, at least one year of home health agency office experience preferred. Proficiency with computer use and software applications including MS Office software products required and experience with EMR software preferred.
Certifications, Licenses, and Other Special Requirements
Ability to work varying hours, including weekends, nights and holidays.
Uses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.
Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Excellent oral and written communication skills, as well as exceptional customer service skills. Must have excellent organizational skills. Proficiency with computers and software applications such as Microsoft Office software.
