- Performs ongoing assessments of resident physical and psycho-social needs and coordinates with nurse to assure quality proactive care as allowed by state regulation and communicates to families regarding resident's personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments.
- Shares on call duties as required.
- Associates degree in health care related field and/or one to two years related experience and/or training; or equivalent combination of education and experience. Refer to state specific regulations for qualifications required to assist with medications.
- In accordance with state law, may need to possess current state certification and follow regulations to maintain certification currency.

- Manages the day-to-day healthcare operations of the community to ensure resident’s healthcare needs are met.
- Ensures residents are treated with respect and dignity and ensures quality care as resident’s healthcare needs change.
- Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Caregiver or Med Tech/Aide in order to meet the residents’ needs and staffing requirements.
- Assists in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and Caregivers and provides training and education to Caregivers on an ongoing basis to include classroom in-services and situation-specific training. In addition, they will also manage the associates schedules.
- Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines.
- Shares on call duties as required.
- Education as required to obtain state nursing license (LPN/LVN or RN) and a minimum of 1-2 years relevant experience.
- LPN or LVN license.

- Our program coordinators plan, coordinate and facilitate a calendar of dementia friendly programs that meet specific needs of each and all residents within the dementia care community.
- You will provide person centered life enrichment programs while engaging residents in meaningful and purposeful programs within the Brookdale community enhancing their quality of life.
- Collaborates in the education of associates regarding dementia care including how to lead programs on an ongoing basis and identifying interventions to address behavioral expressions.
- A Bachelor’s Degree in therapeutic recreation, gerontology, education or related field preferred.
