Initiates individualized Personal Service Plans and proactively manages care and services for each resident. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment. - Ensures and conducts Personal Service Plan reviews and updates, according to Brookdale and state requirements, to ensure residents’ service needs are met. - Assists in the achievement of occupancy goals by monitoring the health, safety and well-being of residents, proactively identifying those at risk for move out. Implements timely problem solving. - Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines. - Implements and maintains Brookdale operating standards, systems and processes. Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences. - Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the community. The Resident Programs Director has the primary responsibility and authority to oversee the quality of life for resident engagement as outlined in Brookdale mission and cornerstones, the Optimum Life philosophy and culture, as well as the Resident Programs Standards of Excellence. The Resident Programs Director serves as a part of the management team to ensure policies and procedures are understood, trained, and implemented. The Resident Programs Director hires, trains, and manages at least one and a half full time associates to ensure all program expectations are in place and all customers involved have quality of life and are satisfied. - Performs all duties in a professional manner and participates in community leadership decision-making so as to have a positive impact on community business operations including management of department budget and staff. - Provides subject matter expertise to the management team regarding associates’ and residents’ engagement and quality of life. Participates in annual resident satisfaction survey initiatives and addresses resident complaints and grievances. - Negotiates and manages community contracts in significant matters such as staffing, contract labor, vendors and entertainers. - Participates in the review of individual residents’ service plans and regularly documents progress. Participate in discharge planning with other members of the management team. - Responds appropriately to resident emergencies, using independent judgment for “in the moment” decisions, calling or contacting appropriate internal and external parties according to the established protocol. May also assist with disaster management efforts and activities during evacuations, (i.e. hurricanes, etc…) - Ensures that Resident Programs meet all state, federal, and other regulations. - Plans, develops and implements creative and exciting Resident Programs to meet the individual needs and interests of the residents within the community as well as through outside trips and events; includes coordination of one-to-one programming for residents who cannot participate in a group setting. - Responsible for meeting the Standards or Excellence for Resident Programs. - Responsible for maintaining Resident Programs’ mission and model which are the foundation of community programs. - Ensures resident welcome and engagement protocols are in place. - Through independent judgment and discretion, manages a volunteer program, including recruitment, training, and supervising volunteers. - Ensures daily, weekly and monthly calendar of events requirements are met. - Ensures specialty programs are scheduled according to the Resident Program Annual Master Calendar. - Ensures marketing, promoting and communicating materials are of professional quality. - Maintains community environment (including equipment and supplies) to support whole-person wellness culture for resident engagement. - Coordinates the transportation of residents to and from events outside of the community, which may include driving a community motor vehicle. - Directly supervises Resident Program Assistants/Coordinators and Transportation drivers as needed. May act in supervisory capacity as manager on duty or in the absence of the Executive Director, if assigned. Responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. - Follows the plan of care, providing treatments and working collaboratively with the members of the team to help meet positive patient care outcomes. - Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care. - Implements current nursing practices following a comprehensive assessment and plan of care. - Provides accurate and timely documentation of patient services to reflect the plan of care. - Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-plan of care. - Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include Hospice Aide, OT, PT, SW, ST, Dietician or Clinical Supervisor. - Uses and prepares equipment and supplies effectively, adhering to aseptic techniques. - Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications. - Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities.
