Provides coordinated skilled nursing care to patients of all age groups, on an intermittent basis in the home. Demonstrates accountability and responsibility in collaborating with the interdisciplinary team to establish and achieve patient goals and maintain high quality patient care. Performs in accordance with physician’s orders and under the supervision of the Manager, Clinical Services. - Assesses home care patients identifying physical, psychosocial and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, call-in logs and on-site evaluations. - Completes OASIS, assessment and visit paperwork according to agency policy. Assures clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members and patient/family/caregiver. - Communicates significant findings, problems and changes to Manager, Clinical Services and physician, and documents all findings, communications, and appropriate interventions. - Demonstrates knowledge and proficiency in identifying, reporting and documenting problems, adverse reactions or possible complications related to patient care. Recognizes and reports life threatening situations and responds appropriately. Recommends and incorporates other disciplines in the care of the patient, as needed, and under the direction of the referring physician. - Participates in the development and implementation of the Plan of Care to ensure quality and continuity of care and appropriate discharge planning. Establishes Plan of Care in coordination with the physician, patient, family and interdisciplinary team members. Incorporates long and short term goals established in the Plan of Care into nursing care, as evidenced by documentation in visit notes. - Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided. - Participates in interdisciplinary patient care conferences and Agency Performance Improvement processes. - Receives referrals from physicians, hospitals, clinics, patients, families and other agencies when needed and notifies agency personnel on a timely basis. - Assures the continuity of patient care and coordination of services through delivery of quality patient care. Performs in a manner that is consistent with current standards of practice and with the Agency’s policies and procedures. Follows all infection control and universal precautions guidelines/standards per agency policy. - Responsible for participating in on-call rotation and emergency call according to agency policy. - Demonstrates commitment, professional growth and competency. Strives to continually improve nursing knowledge base through individual research, reading, attendance at workshops/professional conferences and professional organizations. Manages the day-to-day healthcare operations of the community to ensure resident’s healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as resident’s healthcare needs change.   - Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Resident Care Associate in order to meet the residents’ needs and staffing requirements. - Assists in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and Resident Care Associates. - Provides training and education to Resident Care Associates on an ongoing basis to include classroom in-services and situation-specific training. - Assists in the maintenance of resident charts and review of documentation performed by care giving staff. - As allowed by the Nurse Practice Act, assists in assessing the health, functional, and psycho social status of residents. Initiates individualized Personal Service Plans and proactively manages care and services for each resident. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment. - Ensures and conducts Personal Service Plan reviews and updates, according to Brookdale and state requirements, to ensure residents’ service needs are met. - Assists in the achievement of occupancy goals by monitoring the health, safety and well-being of residents, proactively identifying those at risk for move out. Implements timely problem solving. - Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines. - Implements and maintains Brookdale operating standards, systems and processes. Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences. - Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the community. Ensures communication, as appropriate, on the results of these visits. - Completes documentation as required by Brookdale or state regulatory agencies. - Communicates information in a timely manner to the appropriate person or agency regarding changes in residents’ health/well-being, personal needs, risk management issues, customer service issues, family or outside health care provider concerns. - Assists in keeping the environment safe for associates to reduce the occurrence of Worker’s Compensation claims. - Participates in community events, including family and social functions. - Shares on-call and Manager on Duty responsibilities/shifts as required.   This job description represents an overview of the responsibilities for the above referenced position. Obtains signatures, required deposits and authorizations for move-ins. - Requests assistance of marketing assistant when appropriate to coordinate needs relating to logistics of new resident moves including such things as transference of keys, furniture move-in coordination, unit preparation, new resident orientation and welcoming, etc. Maintains continued contact to reflect concern and interest for their continued wellbeing and satisfaction after relocation to the community. - May provide input regarding the development of promotional brochures, videos and mailings as used in networking referrals and marketing to prospective residents. - Prepares written reports and communicates status of current success in a clear and concise manner.
