Ensures all clinical visits needed are appropriately scheduled to meet the agency’s patient care and service needs and in accordance with physician’s orders, agency standards and regulatory requirements. Tracks and provides feedback to the Clinical Services Manager when scheduled visits are missed. - Coordinates the scheduling of clinical visits for care team in compliance with physician approved orders and clinician plotted visits approved by Clinical Services Manager. Ensures compliance with the schedule. Communicates any requests to change the schedule to the Clinical Services Manager and, once approved, reschedules missed or declined visits. - Assists clinicians with assuring all visits are properly assigned and scheduled on Electronic Medical Record (EMR) devices. - Maintains on-call schedule for assigned care team. Responsible for contacting PRN associates according to established protocol to ensure adequate scheduling of services to meet patient needs. - Responsible for monitoring and scheduling home health therapy reassessment visits to meet home health reassessment requirements. - Facilitates compliance in the electronic medical record for notices provided to the patient or responsible party (e.g. Notices of Medicare Non-Coverage) in a timely manner. - Assures compliance with, and facilitates completion of, service agreements from patient(s) and/or power of attorney(ies) receiving services from the agency. - Fields all calls related to the assigned team patients and routes to appropriate care team members. - Keeps the Clinical Services Manager informed on aspects within the care team specific to EMR scheduling workflow. - Prepares reports for scheduling tracking and participates in agency meetings to ensure patient and associate scheduling needs are met. - Maintains positive relationships and rapport with co-workers, associates, patients, and their family members and community personnel. - Adheres to communication and accountability guidelines established by local and regional administration. - Maintains a safe and clean work area and adheres to agency safety standards. - Adheres to established confidentiality standards as required by Brookdale and HIPAA concerning community and patient information. Oversees and directs the sales efforts for multiple home health agencies within their respective Area. Each Area Sales and Marketing Manager will be responsible for managing, developing, monitoring of marketing plans and budgets. This position works closely with the network Home Health Director (HHD), District Director of Operations (DDO), Regional Vice President (RVP), and all home office and support center associates. Works and strategizes with community and regional support for BSL communities to help support the health and wellness continuum. - Successfully markets and positions BHS programming within assigned territory. Achieves and exceeds all start of care goals internally and externally for the Area in which they are responsible. - Oversees the weekly sales and marketing plans as well as the use of each agency marketing dollars to assure optimal exposure and maximum efficiencies. Coordinates planning and use of marketing budget dollars to create shared efficiencies in media. - Recommends, determines and implements the proper sales structure within their area with Regional teams and the Home Health Director. - Assists in interviewing, hiring, training and coaching of all BHS Home Health Outreach Coordinators (OC). - Oversees and/or completes the marketing plan and budgetary process for the area. Partners with the Home Health Outreach Coordinators and the agency team in the development and modification of short and long-range agency marketing plans and budgets by providing competitor information, data and feedback. - Coordinates the completion of competitive analysis on Top 5 agencies within each market and keeps current on competitive projects and programs in the marketplace within the area they oversee. Understands local markets within assigned region and makes pricing and tool box recommendations. - Monitors marketing and sales performance expectations. Assesses the skills and abilities for each Home Health Outreach Coordinator. Coaches, trains and motivates as appropriate to achieve success. Reviews and analyzes weekly and monthly sales and marketing reports to ensure accuracy of information and to make recommendations for necessary changes. - Visits each agency within the region on a scheduled basis, approved by the Home Health Director and the District Director of Operations. Partners with on-site community marketing teams and Executive Directors in the execution of the sales and marketing programs and strategies to develop and create a health and wellness continuum as defined by the community’s business plan. Leads and participates in team meetings within Region. - Partners with BHS Training and Development to actively lead and participate in sales training activities for their respective region. - Promotes the company and health and wellness continuum in a positive manner and sets the example for company standards for excellence. - Encourages teamwork through cooperative interactions with Regional Operations, and other home office associates. The Brookdale Bench Program will prepare you to assume the Health & Wellness Director role at one of our communities including, but not limited to areas of people management, clinical management, quality care and regulatory oversight when you assume the role of a Health and Wellness Director.   - Responsible for the direct supervision of community-based licensed nursing staff (LPN/LVN, RN), the Med Techs/CMAs (if required by State Regulations) and/or Lead Resident Care Associates and may also supervise Resident Care Coordinators and Supervisor, Resident Care. - Assigns and directs work of subordinates; appraising performance; rewarding and disciplining associates; addressing complaints and resolving problems. - Provides training, supervision, and monitoring of associates in the administration of medications as described/allowed in Nurse Practice Act, to include auditing of medication administration records. - Provides training and education to resident care associates on an ongoing basis with classroom in-services, and situation-specific training. - Supervises the maintenance of resident charts and reviews documentation performed by care giving staff. - As described and allowed in the Nurse Practice Act, assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in their environment. - Manages the health care of residents, including the dissemination of information to families and associates. Ensures that family members are aware of resident's need for scheduled appointments. - Participates in pre-admission screening of prospective new residents. Assures that required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms. Updates assessments as required by policy and as described/allowed in Nurse Practice Act. - Performs on-site evaluations of residents admitted to alternate care environments for treatment, and maintains contact with resident families with the intent of returning resident to Brookdale. - Performs ongoing assessment/observation of residents' physical and psycho-social needs and coordinates with other departments to assure quality, proactive care. - Evaluates residents; documents changes in condition, and notifies executive director, physician, and resident’s legally responsible party/family of resident's condition and reactions. - Communicates with physicians regarding residents' status and condition changes.
