Manages the day-to-day clinical services of a highly complex, multiple product line, or large community. Ensures residents’ healthcare needs are met while treated with respect and dignity, and ensures quality care as residents’ healthcare needs change. Supervises and provides leadership, as well as coaching, to licensed nurses and other direct care staff within the community. May be responsible for leading additional clinical leadership team of five or more members. The HWD level for each community is determined based on the total complexity of the role. Complexity criteria include, but are not limited to, factors such as size, type of product lines, medication management regulations, 90-day assessment requirements, multiple licensure requirements, state regulatory complexity, and skilled services requiring an RN.   - Responsible for the direct supervision of community-based licensed, certified, and non-certified clinical associates. - Supervises, directs, and motivates clinical leadership staff in supervision of clinical care associates and resident care. - Assigns and directs work of clinical leadership team and subordinates; appraises performance; rewards and disciplines associates; addresses complaints and resolves problems. - Oversees and may provide training and education to caregivers on an ongoing basis with classroom in-services and situation-specific training. - Supervises the maintenance of resident charts and reviews documentation performed by caregiving staff. - As described and allowed in the Nurse Practice Act, assesses health, oversees medication administration, oversees functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness, and maximizes the residents’ opportunity to remain in their environment. - Manages and oversees the health care of residents, including the dissemination of information to families and associates. Ensures that family members are aware of residents’ need for scheduled appointments. - Participates in pre-admission screening of prospective new residents. Updates assessments as required by policy and as described/allowed in Nurse Practice Act or other state regulation. - Performs on-site evaluations of residents admitted to alternate care environments for treatment, and maintains contact with resident families with the intent of returning resident to Brookdale. - Evaluates residents, documents changes in condition, and notifies Executive Director, resident’s physician, and resident’s legally responsible party/family of residents’ condition and reactions. - Communicates with physicians regarding residents’ status and condition changes. Provides Physical Therapy services to patients by assisting in the assessment, treatment, program planning and implementation. Implements timely problem solving. - Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines. - Implements and maintains Brookdale operating standards, systems and processes. Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences. - Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the community. Assists the Charge Nurse in day-to-day recording of medical and administrative information in accordance with current federal and state regulations. Ensures medical records are maintained in an informative and descriptive manner. - Records medical and administrative information in accordance with community established documentation policies and procedures and in accordance with state regulations as directed by the nursing management. - Answers telephones, responds to call lights and delivers messages as appropriate. - Maintains a current list of emergency contacts for the assigned unit. - Assists in scheduling and coordinating transportation and communication for resident appointments. - Maintains all residents’ charts in proper sequence, assuring that all records are completed, including vital signs, weights, and heights. Informs supervisor of deficiencies and/or abnormal weight or vital signs immediately.  Audits resident clinical records routinely noting deficiencies and/or discrepancies. - Reports all diet orders and/or diet order changes to the dining services department. - Audits Medication Administration Record and makes out requisitions from the lab book daily. - Mails, tracks, and receives telephone orders. Notifies physicians of tardiness of telephone orders. Processes telephone orders in clinical record after the nurse has reviewed and noted them. - Maintains all charges accrued by residents. - Orders, distributes and tracks all incontinence products and submits report to the Director, Clinical Services.  Establishes PAR levels of medical supplies and assists with routine ordering. Orders and stocks office supplies on the nursing units. - Maintains structure, cleanliness and order on the nursing unit. - Works closely with nursing staff, physicians and residents’ families in order to provide the best care for the resident. - Completes other duties as assigned.
