Manages the day-to-day healthcare operations of the community to ensure resident’s healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as resident’s healthcare needs change.   - Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Resident Care Associate in order to meet the residents’ needs and staffing requirements. - Assists in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and Resident Care Associates. - Provides training and education to Resident Care Associates on an ongoing basis to include classroom in-services and situation-specific training. - Assists in the maintenance of resident charts and review of documentation performed by care giving staff. - As allowed by the Nurse Practice Act, assists in assessing the health, functional, and psycho social status of residents. Initiates individualized Personal Service Plans and proactively manages care and services for each resident. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment. - Ensures and conducts Personal Service Plan reviews and updates, according to Brookdale and state requirements, to ensure residents’ service needs are met. - Assists in the achievement of occupancy goals by monitoring the health, safety and well-being of residents, proactively identifying those at risk for move out. Implements timely problem solving. - Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines. - Implements and maintains Brookdale operating standards, systems and processes. Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences. - Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the community. The Regional Maintenance Technician (RMT) provides Asset Management support for the associates responsible for the general operating maintenance, service and repair, including floor care, of the communities within their region and communities/offices. - Assists with training needs, as necessary, to ensure community maintenance team performs regular maintenance tasks with a strong emphasis on floor care. - Indirectly supervises and oversees community maintenance, housekeeping, carpet care and equipment, and laundry (if applicable). As primary individual accountable for coordinating the patient centered care for the healthcare team, is responsible for providing all services in accordance with the Nurse Practice Act, federal, state and local laws and within the guidelines of his/her professional organization. - Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). - Provides professional nursing care by utilizing all elements of nursing process. - Assesses and evaluates patient’s status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary. - Responsible for the planning, coordinating and delivery of quality interdisciplinary home health care services. Compliance with company standards and federal/state guidelines will be maintained at all times. - Initiates the plan of care and makes necessary revisions as patient status and needs change. - Uses health assessment data to determine nursing diagnosis. - Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. - Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. - Counsels the patient and family in meeting nursing and related needs. - Provides health care instructions to the patient as appropriate per assessment and plan. - Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. - Acts as Case Manager when assigned by Clinical Director/Director of Professional Services or the Clinical Supervisor/Assistant Director of Professional Services and assumes responsibility to coordinate patient care for assigned caseload. - Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes and outcomes as appropriate. - Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. - Communicates with community health related persons to coordinate the care plan. - Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
