- Reviews the quality and appropriateness of the total services delivered and of individual speech language pathology programs for effectiveness and efficiency, using predetermined criteria.
- Utilizes the Community electronic medical record to review the patient’s medical chart, complete forms such as patient screens, physician orders, transition documentation. Assists the Director, Clinical Services in planning, organizing, developing and directing the day-to-day operations of the Clinical Services department in accordance with current federal, state and local standards. Assists in maintaining systems and established policy and procedures.
- Assesses the comprehensive nursing needs of each resident using acceptable long-term care assessment software tools and according to state and federal time frames. Coordinates care plans for each resident. Oversees the day to day care of each resident to ensure care plans are followed.
- Works with other team members to establish appropriate level of care in accordance with Medicare/Medicaid reimbursement levels.
- Assists the Director, Clinical Services in ensuring comprehensive and accurate documentation by nursing service/direct personnel.
- Monitors, manages and provides education on Clinical Systems, programs, and processes as assigned by the Director, Clinical Services. Implements Clinical Systems and programs in accordance with current rules, regulations, and guidelines that govern long term care.
- Monitors the community Quality Indicators and survey reports and provides the Director, Clinical Services recommendations for Success Planning to include conducting and scheduling in-service training classes that provide instruction of clinical skills and required education topics.
- Assists in developing and implementing a Clinical Services organizational structure. Assists in determining staffing needs; recruits, selects, and orients nursing/direct care personnel. Leads programs for staff development. Develops work assignments, schedules duty hours, and delegates authority to supervisory/lead staff.
- Reviews complaints and grievances filed by personnel and/or residents. Reports and investigates all allegations of abuse and/or misappropriation of resident property. Reports suspected or known violations of disclosure of resident protected health information.
- Assists with the coordination of ancillary services.
- Collects, coordinates, and submits MDS data to the state.
- Monitors medication and treatment schedules; works with attending physicians; and provides direct care when required.
- Participates in maintaining the policy and procedures that govern the day-to-day operations of the Clinical Services Department. Possesses knowledge of state, federal, and OSHA regulations, and implements policies and procedures to ensure compliance with regulations and guidelines. Ensures staff compliance with regulations.
- Participates in developing and implementing safety standards and regulations for equipment, supplies, and resident care rooms. Implements safe practices in accordance with state, federal, and OSHA regulations for resident safety, and ensures that policies and procedures in regards to resident safety are followed by staff and by residents.
- Monitors supply and equipment needs and budget. Monitors Medicare/Medicaid reimbursements.

Manages the day-to-day clinical services of the community to ensure residents’ healthcare needs are met. Supervises and provides leadership, as well as coaching, to licensed nurses and other direct care staff within the community. The HWD level for each community is determined based on the total complexity of the role. Complexity criteria include, but are not limited to, factors such as size, type of product lines, medication management regulations, 90-day assessment requirements, multiple licensure requirements, state regulatory complexity, and skilled services requiring an RN.
- Responsible for the direct supervision of community-based licensed, certified, and non-certified clinical associates.
- Assigns and directs work of subordinates; appraises performance; rewards and disciplines associates; addresses complaints and resolves problems.
- Provides training and education to caregivers on an ongoing basis with classroom in-services and situation-specific training.

- Manages the health care of residents, including the dissemination of information to families and associates. Ensures that family members are aware of the resident’s need for scheduled appointments.
- Participates in pre-admission screening of prospective new residents. Assures that required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms. Updates assessments as required by policy and as described or allowed in Nurse Practice Act or other state regulation.
- Performs on-site evaluations of residents admitted to alternate care environments for treatment, and maintains contact with resident families with the intent of returning resident to Brookdale.
- Evaluates residents, documents changes in condition, and notifies Executive Director, resident’s physician, and resident’s legally responsible party/family of resident’s condition and reactions.
- Communicates with physicians regarding residents’ status and condition changes. Ensures residents are prepared for physician appointments and visits.
- Facilitates continuity of care for those residents receiving home health care, hospice services, and other third party healthcare-related services.
- Participates in or leads meetings relevant to resident care issues, such as Service Plan meetings, involving appropriate parties as needed.
- Ensures in-house ancillary medical services such as podiatrist, doctor visits, dental visits, psych visits, lab, X-ray, ambulance, etc. are scheduled and followed through.
- Participates in department quality improvement activities.
